Insurance Coverage

Most major insurance plans are accepted at Calais Dermatology Associates. Gloria Marchand, NP, accepts Louisiana Healthcare Connections Medicaid with referral only.

Copays and deductibles are collected at the time of service. 

Online Bill Pay

We no longer mail paper billing statements. You can securely view your statements, manage your account, and make online payments anytime through the APPatient app. We also accept payments by check or by phone.

To access your statements, download the APPatient app on App Store or Google Play.  Enter the practice URL calaisdermatology.ema.md.

Verify your identity using your name, date of birth, email address, and phone number, then enter the verification code sent by text. Once logged in, tap Billing on the Home screen to view your account.

Preparing for Your Visit

A successful dermatology appointment starts with a little preparation. New patients should plan to arrive 15 minutes early to complete registration forms, update their information, and make sure our team has everything needed before the visit begins.

Complete Your Forms

Medical history forms help Dr. Luckett understand your skin health background, current medications, previous treatments, and any concerns you would like to discuss during your visit.

Bring Your Medication List

Please bring a current list of medications, including prescriptions, supplements, and any topical treatments or skincare products you use regularly.

Arrive a Little Early

If this is your first appointment, arriving 15 minutes early allows time to finish registration and make sure your information is up to date before your visit begins.

Come with Clean Skin

If you are being seen for hair loss or scalp disorders, please ensure your hair is styled in such a way that the scalp and follicles can readily be assessed.
If you are being seen for nail disorders, please ensure you have no polish, gel, dip, or acrylic as these inhibit the ability to adequately assess the nails.

Plan for Photos When Needed

Photography consent forms may be required for certain procedures so we can document your treatment progress and maintain an accurate clinical record.
