Patient Forms
Patient forms are no longer available online. All intake paperwork will now be completed on one of our iPads at the check-in kiosk. We kindly ask that you arrive 15 minutes early, so you’ll have plenty of time to complete your iPad check-in forms. Don’t worry—our team will be happy to assist you if you need help. Our office participates with most major insurance plans. If you have questions about your coverage, please call us at 225-766-5151.
Copays, coinsurance, and deductibles are collected at the time of service.
If you need your medical records sent to another office or provider, we’ll be glad to help. Simply complete the Medical Authorization Release Form below.
